Attention Haulers:
Before any loads can be hauled for DunRite Sand & Gravel, there are 4 requirements that must be submitted before hauling begins.
1: Certificate of Insurance with limits and verbiage as per sample COI below
2: Worker’s Compensation Policy
3: Completed copy of W-9
4: Signed Driver and Carrier Policy Forms
The download above is for Carrier and Individual Drivers to sign. Carriers with multiple drivers should copy as needed. Owner Operators must sign and return both copies. We will be looking at policies to ensure they are conformant to DunRite requirements.
