Attention Haulers:
Before any loads can be hauled for DunRite Sand & Gravel, there are 5 requirements that must be submitted before hauling begins.
1: $1,000,000 Auto Liability
2: $1,000,000 General Liability (DunRite Sand & Gravel listed as additional insured)
3: Worker’s Compensation Policy
4: Completed copy of W-9
5: Signed Driver and Carrier Policy Forms

The download below is for Carrier and Individual Drivers to sign. Carriers with multiple drivers should copy as needed. Owner Operators must sign and return both copies. We will be looking at policies to ensure they are conformant to DunRite requirements.
